CREDIT CARD AUTHORIZATION ACCOUNT

This form allows the cardholder to place orders for delivery or pickup without the credit card being swiped at the time of purchase. It also allows for deliveries to an address other than the billing address for the credit card. By signing this form, the cardholder is authorizing Len-Co Lumber to charge the card for purchases without the card being present at a Len-Co Lumber brick and mortar location.
